Filing IN State Forms ( UC-1 ) for payroll liabilities, need a ICESA/CSV file format

Trying to upload my UC-1 form to indiana's Uplink system and they require an ICESA/CSV file format. How do I convert my saved pdf to this format ?

There's no need for you to convert the PDF document, precisionpulse.

 

QuickBooks gives you an option to create a State SUI file (ICESA type) that can be uploaded on your state website. Please follow these steps:

 

  1. Go to your Payroll Center, then proceed to the File Forms tab.
  2. Click the E-File State SUI link in the Other Activities section. If you don't see this option, click the Employees menu, look for Payroll Tax Forms and W2s, and then click Create State SUI e-File
  3. Select Indiana (UI Quarterly Contribution Report) and click Continue.
  4. Click Continue again, then click Get QuickBooks Data.
  5. Open the Excel file and go to the Add-Ins tab.
  6. Click the drop-down arrow beside QuckBooks Payroll State SUI E-File
  7. Click Start Interview.
  8. Follow the succeeding prompts and enter all required information.
