Vacation is accrued during payment of payroll, so that if someone receives successive raises over time, their vacation accrual remains the same. Thus if someone had several hours accrued over the course of several raises that would account for your situation. You are only required to pay out the accrued amount.

What @LeithG mentions about paying based on the amount you're seeing instead of the hours is correct. In QuickBooks Online Payroll, the amount is the more accurate of the two, and the hours being the way they are could be for a number of reasons, including changes in pay rates or even the policy set up for the employee. If you'd like to verify the vacation policy is correct, feel free to do so by navigating to Payroll, then selecting the employee name from the Employees tab, then Edit employee.
