If the company address does not display on a T4 slip and T4 slip printout, follow the steps below.
Resolution
As per the CRA, a company address does not need to display on the slips. From the CRA information page:
“Enter your operating or trade name in the space provided on each slip. This should be the same information that appears on your statement of account. If you would like to, you may also add your business address in this space."

Display a company address
To have the company address display on a T4 slip and a printout, enter the company address on line 3 of the T4summary form. The company address carries to the slip.
