If you are paying it, then it is an expense, enter the payment and select the expense account you set up for fuel or fuel surcharges
If you are charging the customer, create a service item for it, select an income account on the service item screen and use it on an invoice or sales receipt. I would create a sub income account called fuel surcharge and use that
