Please open QuickBooks in a private window and see how your report shows. Private browsing disables the cache from saving your browsing history, and it's a good initial step to check issues in a browser. If the format goes back to normal, you can go back to your regular browser and clear the cache. If not, please try other browsers to isolate the case.
