Why do banking transactions wind up in my credit card file? How do I fix this?

I'd like to know what meant about the bank transactions wind up your bank files? Is your bank transaction cannot be matched? Any additional details would really help me fix this.

Why do banking transactions wind up in my credit card file? How do I fix this?

For example:

QB shows 3 entries for Verizon telephone, all on the same date, all for the same amount.  Only one of these appears on my bank statement.  This type of duplication usually happens every month.

 

QB shows, constantly, charges attributed to Gulf Oil in amounts ranging from $2.17 up to 66.85.  I do not use Gulf Oil.  Theses charges do not appear on my bank statements.  There usually are two or three Gulf Oil charges each month.
