How do I generate a CSV file for IL-941 to upload to MyTax?

I am looking to efile my quarterly IL-941 to Illinois's MyTax web site since Quickbooks only does the annual.  MyTax is requiring a CSV file with 4 columns, Employee Name, SS#, Total Income & Amount Withheld.  It's hard to believe that it is impossible to do this with Quickbooks.  I know I need to generate a report and then send to excel or use the Tax Form Worksheets in Payroll center, but I can't find one that works.  Payroll center separates the employee's name into two separate columns while choosing and employee report adds the first and last name together along with the SS#.  Am I missing something?  Please help.

I don't want you to feel misled. I can see how exporting a report into the exact format you need in your Excel CSV file would be very beneficial for your business. However, you'll need to edit the information in the CSV to best fit the format for the MyTax website. This would be the easiest way to get the information you need to import that CSV file into the MyTax page.
