Hi @mbalazic,

I'm here to extend a hand on your concern. Before we can proceed, I'd like to ask for additional info on what you want to do.

If this is about the submission of that form you stated, there are specific QuickBooks Online Payroll versions that can submit it for you. You can check this article for additional details: Tax payments and forms Intuit payroll submits for you. Scroll down to the Full-service versions of payroll section to see a list of versions that have this feature.

In case you have a different version than what's on the list, I suggest you visit your state agency's website to know how you can submit the Form WTH10004

I'll be leaving this article here as well: See Tax Payments And Forms. It has a list of steps on how you can locate submitted tax forms.

If you're referring to something else, I'd appreciate it if you could provide more details. Post a comment below, and I'll be sure to get back to you.

The form I referenced is correct and is identical to the one I print from Quickbooks each month.  The problem is that there is an electronic signature already on the form when I print it.  It should be blank so it can be signed by me before submitting.  The signature is a name with no connection to our company.

I have been instructed to submit this form my self because Quickbooks does not submit the Oklahoma forms.  Only the Arkansas ones.

Let me step in, @mbalazic.

I'm here to share some information about state filings in QuickBooks Online.

In Intuit, we have assigned filers that taking care of the tax forms on your behalf. If you’re an assisted payroll, this might be the reason there’s an electronic signature printed on your form.

Here are some reasons why QuickBooks doesn’t submit them:

  • Some state agencies do not support third-party filers.
  • You'll need to use original or scannable forms provided by the state agency.
  • Combined state and federal E-filing is not supported.

You can visit the state website to get a blank form. Since there are already information you need on each line of the form, you can copy the data manually. And then you can send it directly to your state agency.

You can also contact our Payroll Support team to assist you further. They can modify the details for you. This way, you can print the form without other names stated on it. Here’s how:

  1. At the top right, click the Help icon.
  2. Click Contact Us.
  3. Under What can we help you with?, enter your concern.
  4. Click Let's Talk.
  5. Scroll down and choose to Get a Call.

I’ve attached some resources for your reference on which states offer electronic filings and extra information to ensure compliance with state payroll tax regulations:
