Hello there, @slcarriker.

I hope you're having an awesome day so far.

The issue as you've explained it is a common one that's created when important work location information isn't entered during your QuickBooks Online Payroll setup. Without a work location created, it will use the default address in California (Intuit HQ). In order to have these forms removed from your Payroll, please reach out to our terrific Phone Support Team using the instructions below:

Contacting the QuickBooks Support Team

  1. From QuickBooks Online, click the Help (?) icon in the top-right.
  2. Select Contact Us.
  3. Enter what you need in the How can we help? field, and click Continue.
  4. Click Get Callback, enter your contact information, and select Call Me.

These instructions are also available from our guide on contacting QuickBooks Online support for your benefit. I've included a brief video demonstration of this below:

With their help, once the work location has been assigned, the team can run some recalculations on your taxes owed to eliminate the due forms. Please know you can always get in touch with me here anytime you need a helping hand, I'm here to ensure your success. Thanks for coming to the Community and take care.
