Filing Enrollment Instructions
You must enroll with the Florida Department of Revenue (DOR) in order to e-file Form RT-6 and e-payments. You should enroll as soon as possible and no later than 30 days before your first payment is due. You will create a User ID and Password, as well as enter bank account information. These are not needed to file from Intuit but are required to register and access the Florida Website. You'll need the following information on hand: - Taxpayer ID Number (7-digit UT Account Number) - Federal Employer Identification Number (FEIN) - Official Business Name - Contact Person's Name, Address, E-mail address, Phone and Fax Numbers - Financial Institution Routing Transit Number (RTN) - Financial Institution Account Number - Financial Institution Account Type (Checking or Savings) To enroll in Florida e-Services: (1) Go to Florida DOR Web site. If you haven't registered to obtain a Florida UI Account Number, register first, and then enroll. (2) Enter and verify all requested information. (3) When you have completed your enrollment, you must electronically sign the enrollee authorization and agreement. This enrollment becomes a legal and binding agreement once submitted. (4) After your information is validated, an Enrollment Confirmation page appears that includes confirmation of your enrollment and your User ID and Password. Print this page for your records. Note: If you get a new Taxpayer ID Number, you will need to re-register and re-enroll using the e-Services Web site. For additional information, contact e-Services Unemployment Tax Customer Service Monday through Friday, 8 a.m. to 5 p.m., ET at 800-482-8293. You'll find a complete contact list on the Florida DOR Web site.

Paying Penalty & Interest
1) On your RT-6 Form, enter the Penalty (Line 6) and/or Interest (Line 7) amount. 2) As you are submitting your e-fileing and e-payment, this information will be displayed on the Liability Payment screen. 3) Select the payroll expense account you want to apply to the penalty or interest amount. 4) Click E-file and E-pay to continue.

In 3-5 business days call 850-488-6800, give them the confirmation number and they can give you the account number over the phone.
