Welcome to the Community, @weedcokim. I understand that you've already made sure that you've entered your new employee's W-4 in QuickBooks Online. But it doesn't hurt to double-check it.
This time, I'd recommend checking the USPS database using the USPS tool to look up the city, state, and zip code combination of your employee's address to make sure it's accurate.
The website has a searchable database that uses the same information to find the corresponding zip code of a given street address and city.
