To start, use a supported, up-to-date browser (Google Chrome, Mozilla Firefox, Microsoft Edge, or Safari 11 or newer). Currently, Internet Explorer 11 is an unsupported browser for QuickBooks Online (QBO).
If you are already using a supported browser, the missing Gear icon might have been caused by a web browser-related issue. Stored cache for a long period can potentially affect the views and performance in your QBO account.
To fix this, access your account via a private or incognito window. This mode prevents browsing history from being stored. Here are the shortcut keys you can use to access a private/incognito window:
Google Chrome: Ctrl + Shift + N
Internet Explorer: Ctrl + Shift + P
Mozilla Firefox: Ctrl + Shift + P
Safari: Command + Option + P
If you can find the Gear icon, you can clear the cache in your regular browser. Doing that helps deletes any necessary contents stored and allows your browser to function more efficiently.
