To start with, you'll need to change your last name using the QuickBooks Online test drive to see if it works.
If you still get the same result, let's perform some troubleshooting to see if this is a browser issue. Too much data stored in the browser may be the cause of the unexpected behavior.
First, switch to a different browser and log back into QuickBooks Online. You can use any of these supported browsers.
Google Chrome: Ctrl + Shift + NInternet Explorer: Ctrl + Shift + PMozilla Firefox: Ctrl + Shift + PSafari: Command + Option + P
