Let me share a few things about the Audit Trail report in QuickBooks Desktop. When a user, makes changes or deletes a transaction, this won't overwrite the last modified date. It'll just create a new line on the report.
Since you're getting this unusual behavior, I'd recommend attempting to verify/rebuild your company file. This way, you can check if there are any data issues and fix this using the tool. You can refer to this article for the detailed steps: Verify and Rebuild Data in QuickBooks Desktop.
Correcting any data issues will surely get you back to business again.

I did a test run of the 'Verify & Rebuild' processes on a copy of my file - it did not fix the problem of the latest user and date overwriting the most recent user and date in the audit trail. Please see the redacted screen shots from the audit trails we ran before and after the process.
Verify and Rebuild Data did not fix the problem. Not only did it not correct the prior overwrites that happened on or before 4/4/2020 and continued to be seen after that; it is still occurring to the file after running the rebuild as recommended. Please see the attached file with redacted samples of the audit trail reports I have saved that show the issue still exists.
