I have tried on Chrome and Explorer with the same results.

Let's get this sorted out.
Clearing your browsing data will help clear the data that has been stored over some time. This will allow QuickBooks to load as a new page and remove any errors or glitches that may be standing in the way. Here's how:
Note: Let's try Google Chrome since QuickBooks was made around Chrome.
3. Press Clear browsing data.
4. Choose the "Time Range" for All time, then click Clear data.
It's that simple. I also recommend looking into this article about importing custom form styles.
