Why does my check after printing look like a pay stub when we are paying by paper check?

  3. Choose Print setup at the bottom.

  5. Make the necessary changes, then hit No, continue setup until finish.

I checked your screenshot, and I can see that the check doesn't come from QuickBooks Online (QBO). That said, I highly suggest contacting the Internal Revenue Service (IRS) to check why the check was not cashed.
