How do I make the standard P&L show totals by each month and then total by year, ie. column headings Jan, Feb and so on?

You'll need to customize the Profit & Loss report to show the amount each month and the total year. Let me walk you through the steps: 

  1. Click Reports.
  2. Go to Company & Financial
  3. Select Profit & Loss.
  4. Click Customize Report
  5. Select the Display option.
  6. Change the date range.
  7. Choose Month option on Display Columns by drop-down arrow.
  8. Click OK
