Currently, there's a new update for the location of the + menu icon in QuickBooks Online. For the new workflow, you can find it on the upper left side. I've attached a screenshot below for your visual reference.
I've also added a QuickBooks Blog about some new features and improvements in the product, like how to simplify credit card payments with the new workflow, get organized and stay compliant with sales tax filing, profit and loss report enhancement, and create custom roles: What’s new in QuickBooks Online: November 2019.
