Adding the contact information only allows you to input one. That said, you can choose between your office phone number or your personal number.
I'd recommend using the one that you usually used for your business. Also, you can use the US phone number format.
I'm going to take note of this and pass along to our developers. This way, adding two contact information in the form might be given a chance to be added in the next product updates.
