Let's go to the settings to check all the available templates, @Jeff N.
You can go to the Gear icon and choose Custom Form Styles. On that page, you can view all the invoice templates that you have. It's also possible that the template that you're referring to was deleted. You can check your audit log to check who has recently logged into the account.

In case the template was deleted, all the transactions associated with that style, the default style will automatically be applied. You can go to the Reports menu and search for the Audit Log report. Here’s more information about audit log: Use the audit log in QuickBooks Online.

Hi MaryJoyD, thanks for responding. All my custom form styles are deleted and nothing in the log explains this. I have pdf prints of my previously used custom invoices over the last year but now everything is reverting to Standard Master when I go back and preview the same invoices with QBO. Any chance I can get them back or do I need to rebuild them again?

Hello Joesem M, my question was about the bug that deleted my custom templates 2 days ago with the forced prompting to add payment information text. I know how to make custom templates, but the point of a template is that they can be used over and over and don't spontaneously disappear.
