You can include the penalty when you process the liability payment in QuickBooks. I've got the step by step instructions here:

I need to create a payment that appears in my PAY Liabilities window that I can select then E-PAY as I normally would pay a scheduled liablity. So far I have done this in the past but the procedure has been lost.

We can no longer select that liability since you already paid it. To fix this, we'll have to check the e-pay status of the liability first. I'll guide you how:
If the status of that payment is Agency accepted, this means that the e-payment was successfully processed by the agency and the transaction is already complete. Then, we'll have to pay your penalty directly to the EFTPS website.
If the status is still Submitted to Intuit, we'll have to void that liability check. You can follow the steps below:
Once done, you can now process the liability payment and penalty by following the steps above provided by RenjolynC. Aside from this process, we can also include the penalty to your next liability payment if you wish to.

We're "beating around the bush" here. I have no payroll payments due at this time. All of my previous payments have been accepted by the agency. When CA was a Credit Reduction state we had to make an additional payment every year starting in 2017. My accountant had an assistant at that time that was able to send me detailed instructions to add that payment to the Pay Taxes and Other Liabilities window so that I could select it, then select View/Pay button and proceed with the normal EFT payment. That info has been lost. How do I add a payment to the Pay Liabilities window???

The steps provided by my colleagues above are accurate for recording penalties, however it seems your case is unique.
