From the Merchant Service Center, choose Set Up Recurring Payment and select Complete authorization form under Quicklinks, or directly click this link to download the form: https://merchantcenter.intuit.com/rebill-resources-9.81.0-rel-3/pdf/AuthorizationForm.pdf.
Since Payments is showing the message prompt Access Denied, make sure to enable the browser cookies so the system could allow you to perform any activities within the account. For other troubleshooting solutions, check out this article: Problems accessing or using the Merchant Service Center.

I'm glad I was able to help you find a downloadable copy of the form. You should now be able to get written authorization from your customer.
