Foreign medical expenses

My client received medical treatment outside Canada and got a receipt. There is a service date and payment date. Which date should I use to exchange the expense into CAD? Appreciated

It is my understanding that the relevant date is the date of payment.  And we don't have to use the calendar year; it's any 12 months ending in 2022.  So if there are unclaimed 2021 expenses that can be brought into the 12-month envelope, go for it.  The only problem is remembering the following year what was done.
