Employers in the state of Vermont who don't offer insurance to all of their employees may be required to pay a Health Care Fund Contribution.

Report the Vermont Health Care Contribution assessment to payroll
If you're required to complete Form HC-1, you must send QuickBooks Payroll a copy of your completed form by the 10th of the month following the close of the calendar quarter for processing.
You can access the HC-1 form from the agency's website.
We will use this form to:
- Adjust and file your Quarterly Withholding Tax Filing Form WHT-436 with the state of Vermont.
- Collect and pay Health Care Contribution amounts due based on the HC-1 Worksheet provided.
Email your completed Form HC-1 to PayrollTax_OperationsEservices@intuit.com. You will receive email confirmation that we have received your form and applied the Health Care Contribution to your payment and filing.
Keep your copy of the completed form HC-1 for your records.
