Thank you for raising this. I am not aware of any reports or investigations running for these particular topics, however there can be multiple different settings and/or factors that can affect the experiences described when using QuickBooks Online. For example, a pending direct feed application can affect the application of existing bank rules when coding transactions, or a GST code may have been switched off as 'available' in the GST centre and need to be re-enabled. Visual errors and other issues can often be quickly fixed by clearing your browser's cache. I would first suggest clearing the cache and if you are experiencing errors across several client files would recommend you reach out to our support directly so that our team can get eyes on what you are seeing, make sure nothing been lost in translation and probe further into each individual matter to ensure all options are provided and escalate if required:
