Only left with category, description, cost and tax. Have tried to find a setting to fix but left with no options, even the drop down at end of row only offers the colums already there.

The item details may not be visible on the purchase order because the setting in your account preferences is turned off. Let's enable it again to resolve the issue.
Here's how:
Once done, try creating a new purchase order to see the result. If the items dropdown is still missing, sign out and sign back in to refresh the page.
