There are three types of customer statement forms you can use.
Based on the information you've provided, the closest one to use is the Balance Forward since it displays the amount due to your customers. It's important to note that customer statements only show the values of your sales transactions and cannot be customized directly. If you need to change the dates of the transactions, you can manually edit them by opening them one by one.
