I'd like to verify, are you subscribed to QuickBooks Online Simple Start? If so, we have an ongoing investigation about the missing Add user button. Our engineers are aware of this situation and are working on resolving it as soon as possible.
As a workaround, you can upgrade your subscription to Essentials to add the user.

We are having a weird set of issues on this add user page.
1. It will not allow us to type into the first and last name fields more than one character before the cursor jumps to the end of the email address field
2. It does nothing when we click "add user" green button at the bottom of page. Page just "bounces" back to top but doesn't add user.
