I would like to know why QB online failing transactions as per the explanation above.
There is a case like mine for the QB Desktop. Not yet solved. There is a chance the developers decided to re-use the core software routines in both QB products meaning the problem It will appear in both products. Please see below the link to that case.

Hello SeaView, it should bring up two transactions into QuickBooks even if it is the same date and amount. What is the date of the transaction? because if the date is today for example then it its worth waiting for 24 hours usually it takes time to show up in QuickBooks.
