From what you're describing, it sounds like you're referring to the 10% Temporary Wage Subsidy Self-identification Form for Employers. Because this form was a temporary filling, you won't be able to generate it directly from QuickBooks Desktop Payroll. However, you can use the information from the PD7A form in the program to help you fill out the PD27 form before submission. Since you can't access the PD27 form in the payroll module, you'd have to download it from the CRA website: PD27 FORM
