Is there a template that shows the import spec from QB Online? We want to be able to import hours by code to pay through QB Online Payroll.

Currently, there's no template showing the import specs in QuickBooks Online Payroll that enables you to import hours by code. However, you can look for a third-party app that integrates with QuickBooks in our QuickBooks App Store.

Currently, importing hours to QuickBooks Online is unavailable. Fiat Lux - ASIA and GebelAlainaM provided a great option you'll want to consider.
The App Center has all kinds of third-party applications to choose from. Get a better comparison of these apps and see if one of them meets your needs.
For service code, only allows you to assign one workers compensation code to each employee, which typically is their main line of work. If an employee needs an additional Worker's Compensation code assigned to hours worked, you can edit and add the code manually to the paycheck. You just need to make sure that the rate won't change for two different codes.
Just a heads-up, setting up a maximum allowed accrued hours on your employees' sick policy automatically stops QuickBooks in accruing hours when they reach the limit. Though, it's not set up as a per year feature. It will just stop when it reached 24 hours.
