For further clarification, here's the bit of contract and what our clients require:
By checking the box, Contractor agrees to ensure compliance with Governor Inslee’s Proclamation 21-14 and amendment 21-14-1, COVID-19 vaccination requirement or approved exemption status from the COVID-19 vaccination for all individuals performing work for the District by the contractor and/or sub-contractor. Verification of vaccination/exemption records will be maintained by the contractor.
Please be aware that the District retains the right to inquire into compliance with the verification requirements at anytime, to include requesting a copy of the verification plan and reviewing your company’s vaccination verification records.

I can see the value in adding attachments or vaccination cards to each employee in QuickBooks Online. I'm here to give you details on adding this information to them.
We can only keep track of each employee's vaccination status by adding some notes. While adding a document as proof of their vaccine information is unavailable in QuickBooks Online.
To enhance your experience, I recommend sending feedback to our product developers for this function in QuickBooks Online.
To do so, please follow these steps:
1. Select the Gear icon.
2. Select the type of feedback you want.
3. Leave your feedback or product suggestions in the box below.
4. Then, to submit feedback, click Next.
You may track the status of your summited feedback on our QuickBooks Online Features Requests page.
Furthermore, you can run a report to identify the employee vaccine status and export the report to excel.
Here's how:
3. Select Change Column to edit the row/column. From there, make sure to a checkmark on the Employee, Address, Phone number, Note, and other detail you want to add.
4. Hit Run report.
5. Lastly, tick the Export button beside the Printer icon. This way, you can export them to excel or PDF.

Is there a way to then export a report with all employees showing the notes field and thus showing a list of all employees vaccine status?

We can run two reports to show the vaccination status of your employees. One of it is the Employee Contact List which was mentioned by my colleague @ChristineJoieR above. Another way to track their status is by generating the Employee Details. This contains the employment info, payroll details, notes, and more.
