We haven’t received reports similar to this from other users. With this, it's possible that you're experiencing some browser hiccups.
Let’s perform some troubleshooting steps to check if this error is caused by browser cache. You can use a private window or incognito mode to open your QuickBooks Online account. It won't use the data from the existing webpage cache.

Once done, try to import the .CSV file again. If this works, you’ll need to clear your browser’s cache and cookies to refresh the system. You can also switch to other browsers to see if the problem is with the one you're currently using.
If the issue persists, you’ll want to try change the file extension to lowercase. For instance, .CSV to .csv. Feel free to read about the correct format and its restrictions.

Thanks for the auto-reply. I wish we were having a "CSV" issue, that'd be easier to fix. "This however is a CVS filling status validation issue."

I've come to provide some updates about the CVS filing status validation failed error when correcting an employee's tax withholding.
Our product engineers received reported issues about the CVS filing status validation failed error upon updating W4 info. Rest assured that they're diligently working on a fix to this issue.
As a workaround provided by our product engineers, you can run the Employee Details report to confirm tax details are correct. Please know that payroll taxes will still calculate correctly despite the issue.
Also, to ensure you'll get an update about the resolution status, I recommend contacting our QuickBooks Support Team. This way, they can also add your company file to the list of affected users and provide this investigation number for easy tracking: INV-74089. To reach them, you can follow the steps below:

RESOLUTION
Please check the following:
Verify that the CSV file is formatted the same way as our sample CSV file.
Make sure the "Username" column has a header defined.
Make sure the username field is not blank for any users.
Make sure the delimiter used in your file is a comma. Note that Excel uses your operating system's default delimiter.
Eliminate any typos, namely incorrectly spelled column headings like 'username' or ' phoen1'.
Ensure that the file does not have the wrong extension. It should be .csv.
Open the CSV file with Wordpad or Notepad++ and delete any empty lines at the end.
Ensure that the file is encoded in UTF-8 without BOM. To do this in Notepad++, follow these instructions:
Select Encoding in the Notepad++ toolbar.
Select Convert to UTF-8 without BOM.
Save the file, and try to import it again.
Edit the sample CSV file directly and add your user attributes for upload.
