Intuit debit card

I received a Intuit debit card for my business.  I did not open any account with Intuit that needs a debit card.  I have called several times and can not get an answer.  Nothing came with the debit to explain anything and what account this maybe part of.   

 

I believe this was done fraudulently. Can someone help?

Intuit debit card

What is the best way to contact support regarding this specific issue?  I spend two hours last Wednesday trying to get answers to why I received a debit card and never got a good answer.  I called the number on the back of the debit card, also the Green Dot number I got from a Quickbooks support.  Nobody could give me anything as to how, what , and when the account was opened and a debit card issued because my information could not be verified.  If my information can not be verified someone has changed it.  I am really frustrated that someone opened this fraudulently and now I can't get any information on my own account.
