Is it possible to remove 'please detach top portion and return with your payment' from the statement? This harks back to the 20th century when people used cheques!

After reviewing your response, it seems that the attachment is not attached. No worries, I'll help you upload the screenshot successfully. This way, we can further examine the statement and provide a solution. 

Here's how: 

1. On the Reply to the message page, click Choose File or Photo button. 
2. Pick the appropriate file and hit Open. 
3. Select Done, then Post.

The steps below will surely help you get the thing you want in QuickBooks. 

1. Go to the Gear icon.
2. Select the Custom form styles under Your Company.
3. For the standard template. Click Edit or double-click to open it.
4. In the Design tab, click Edit print settings.
   
5. Uncheck the Fit printed form with paystub in window envelope box.
   
6. Click Done to reflect the changes.
