Merchant services fee schedule

What is the merchant services fee schedule for credit card processing with QB Pro? There are two monthly charges, plus a per transaction fee.

Merchant services fee schedule

Here is the fee schedule from a while back. If it were not still in effect you would think they would change or delete it
