If you use our BRC Donor Statements - QB Online application, which uses your QB Online data to create donor statements, you can include your own custom text in both a letter like format and for the statement footer, where you might put the legal disclaimer information including your business name and non profit status and tax ID.
We also offer the similar BRC PositivePay file Creator - QB Desktop for Desktop.

Let me give some insight regarding customizing customer statements.
I can see how it would be useful to have the ability to add another note field to your customer's statement. Currently, you can only customize what appears in the activity column and whether to show the aging information at the bottom of the statement.
Please know that I'll pass your idea to our Product Development team to see if this feature will be available for future updates.

Thank you...I am not looking t add "another" note field. The note field (box) does not show up at all when previewing and printing a donor statement.

I appreciate you for getting back to us here in the Community. Let me share some information about generating a statement in QuickBooks Online.
Currently, the option to add a note or memo box in statements is unavailable. As a workaround, you can utilize the footer field to include the required information.
To add details, please go to the Custom form styles section. You'll need to find the Master template to be able to make changes to the footer info.
Here's how:
Once done, access a customer statement and check the print preview. The information will show at the bottom part of the page.
