I want to make sure you're able to use your default template when creating an invoice in QuickBooks Online.
Based on what you've described you may want to check a setting that automates older info that had been entered before. Here's how:
1. Click the Gear icon in the top right corner.
2. Select Account and settings.
3. Choose the Advanced tab on the left.
4. In the Automation section, make sure the Pre-fill forms with previously entered content option is turned off.
5. Click Done when you're finished.
Give this a try and then attempt to create a new invoice once more. Please don't hesitate to reach back out if you have any other questions. I'll be here to lend a hand.

Last screenshot. This one shows that I have my custom form selected. It is just not the template that populates when I select a new invoice.

Referring to the Pre-fill forms with previously entered content option, you'll want to make sure it is turned off.
In addition to turning that off, I recommend clearing your browser cache. The cache is in place to collect frequently used data to make loading web pages quicker and easier. If it becomes too full it can potentially cause viewing and performance errors within the system. Clearing the cache will remove anything that is preventing the correct template selection from loading. Here's how to get this done in Chrome for example:
1. At the top right, click More (three vertical dots).
2. Click More tools and then Clear browsing data.
3. At the top, choose a time range. To delete everything, select All time.
4. Check the boxes next to "Cookies and other site data" and "Cached images and files."
5. Click Clear data.

I followed all of the instructions, and I am having the same issue.
I have a custom invoice set as the default, QBO is defaulting to the "Modern" style invoice.
Our customers have set billing formats and our invoices are being rejected.
We are manually changing every invoice (very time consuming), are there any other suggestions?

Since nothing mentioned above did the trick, I recommend reaching out to a member of the QuickBooks Online Support Team. Agents have specialized tools, like the ability to share your screen, to take a more in depth look as to what's causing this issue with the invoice layout. They will also be able to get this issue escalated for you if need be. Here's how to get in touch with the team:
1. Click the Help (?) button.
2. Select either tab to get started:
- Assistant: Get quick, personalized answers. Select a suggested option, or type a question or topic you need help with. If you decide you need further help, you can still Talk to a human.
- Search: Search the QuickBooks Online knowledge base directly, or select Contact Us and choose a way to connect with us:
a. Start a chat with a support expert.
b. Get a callback from the next available expert.
