You can use a 3rd party payment gateway to integrate with QBO. You don’t need to create invoices beforehand for your patients to pay. The patients will simple enter the amount they would like to pay. The solution provider would create a checkout page link for them. It wouldn’t be a portal per say, because there wouldn’t be anything to log into, just a stand alone checkout page, but they will do create it for your patients. First, they would help to get a merchant account set up so that the payments can flow into the proper place.
