I appreciate the screenshot and details. I'll give a brief overview about the line header information.
The line with the report dates will always show on the report view in QuickBooks. One way to remove it is to export the data to Excel, then manually delete the row (it is exported as a single row type).
After exporting it to Excel, simply right-click on the line then select Delete row.
