As long as the format and mapping are correct, you can upload your chart of accounts in QuickBooks Online (QBO) without issues.
Please know that QBO imports data in a specific structure and naming convention for the Type and Detail Type columns. Account types and Detail types determine the data shown on financial reports like the Balance Sheet and Profit & Loss statements. Make sure everything is accurate.

Once logged in, check your chart of accounts. If everything is right, get back to your regular browser and clear its cache. Doing this helps the program to run smoothly. You can also use other supported browsers as an alternative.

I've tried on incognito after clearing cache and following your instructions. Unfortunately the same result occurs.
I've tried both formats below with the same result, that is, none are uploading as sub accounts.
Main Account: Sub Account
Main Account:Sub Account
Quickbooks does properly remove the prefix account name Main Account on both uploads but it does not nest the Sub Accounts under any of the prefix accounts.
