Thank you for that suggestion. I contacted support prior to posting about this in the community. During screen share the agent could see what was happening, agreed it did not make sense, but did not open a bug report. Instead, she suggested I wait 24 hours and see if anything changed. I thought this was lame but went along. To this day the behavior has not changed.
There is a workaround. Clicking "Details" instead of "Add" button brings up the split transaction with correct amounts where I can fill in payer name and date (because QB does not remember those from the previous screen...) and save it. The end result is correct transaction in the bank register, but this is a workaround.
The Add button no longer fills in the correct numbers in the split transaction for PayPal. As described in the example my first post, using the Add button subtracts the PayPal fee twice.
This randomly started happening in December. This is new behavior and it is wrong. For every other account (and previously for PayPal) clicking "Add" added the transaction to the bank register with correct numbers.

Allow me to help you with your concern about PayPal fees.
As previously mentioned, we are currently investigating the issue of
Split PayPal Transactions are not categorized the way they were added to the register in for review screen (class, amounts, categories, and line totals) in QuickBooks Online (INV-95009). Please be assured that our engineers are diligently working to fix this issue. If you haven't already contacted our phone support, I suggest doing so. This will allow them to add you to the index of affected users and notify you via email once the issue is resolved. You can follow the steps given by the colleague above.
Moreover, as a workaround, you can manually edit the class and categories in the register tab to be able to work efficiently.
