Michigan UIA form says available soon but has not been available for two weeks. when will it be?

Just asked the same question.  I called about it last week and the rep said there was some type of update done and that the form should be available soon.  Would love an update on this.

The Michigan UIA 1028 is currently unavailable. However, our product engineers are working on this. Since there is no specific time yet as to when it will be available, I suggest contacting your State Agency in Michigan.

Is there a way to figure out how to complete the form manually?

To complete the form manually, you can visit this link to fill out the form and fax it by calling the UIA's Office of Employer Ombudsman to file. Rest assured, there are other options on how to file on this web page. By contacting your State Agency in Michigan, they'll help you with the process.
