If you're using a Do it Yourself payroll, you'll have to file the NC-5 directly to the state website: NCDOR.
Otherwise, if you're using a Full-Service Payroll, I recommend reaching out to the Payroll Support Team. This way, they can verify why it wasn't submitted and help you file the form.

I have assisted payroll and quickbooks is not filing the NC-5 form for me and will not let me create the form using the system. It tells me that quickbooks does this for me which is not true. I do not know what values to put in the fields to file it myself.
