but shouldn't qbo file/prepare the NC5 for us? according to the official intuit payroll forms list (link below), it shows that "Quickbooks online Payroll Core", which is the service we currently paid for, should and would file "State WH (withholding)" forms for us?
We paid extra $12/month for this extra NC state. If qbo does not do this, what did this $12/month pay for?

Yes, QuickBooks Online Payroll Core handles the filing for state taxes, @william75. However, NC-5 is a payment coupon used for actual check payments made to the state for both Quarterly and Monthly deposit frequencies.
As QuickBooks makes payments through ACH Credit, Form NC-5 is not automatically generated. Thus, if you receive a notice for a payroll tax filing, you can forward it to us so we can provide the required proof of payment, filing, or correction.

Because the NC-5 form itself says clearly, "This report must be returned even if no tax has been withheld." So this is more than just a payment coupon.
For the peace of mind, why can’t qbo just file this like that of other states e.g. CA DE9 instead of passively dealing with it when receiving unfiled notice from the government agency?

Again, this NC-5 is not just a payment coupon but "This report must be returned even if no tax has been withheld." per the official NC instructions.
