I want to make sure we're able to help you out with your employee setup. However, I need to be sure which form are you pertaining to. Are you referring to the employee's W-4 or do you need a new hire report? Any additional details will greatly help.
If you need to get the employee's W-4 information to enter in QuickBooks, you can use Employee’s Withholding Certificate. This way they can enter their personal data such as name, address, social security number, dependents, and adjustments. Your employees can also enter this info when you invite them, but you are required to keep a copy of this for your records.

Please be aware that if during a new hire's self setup in workforce, the new employee accidentally checks the new hire box that QB will not do the new hire reporting with state.
For QB to automatically file the new hire report the new employee should not check that box.
Unfortunately the employer has no way of knowing that the new hire checked the box and the new hire reports were never filed with the state.
