Ok, we were seeing it connected via Express Web Connect for Chase and Amex on the server side. Doesn't make much sense to me - all I can think of is if you have a second company file connected to those accounts using the same realm somehow? Like if you made a copy of the company file at some point and are using both simultaneously? I don't think it could handle that - has to be different realm numbers.
In any case, we went ahead and disconnected them. Try setting it up again and let me know here how it goes. We did notice that sometimes the server side software will present a dialog (in the built in web browser inside of QB Mac) that lets you disconnect the server connections. I didn't know it could do this but if you see that feel free to disconnect them.
@IntuitBrooks Thank you for staying on this, finally we have success. I was able to get all the cards connected again. Of course still no ability to connect one bank (AMEX) with 2 different logins. Hopefully that terrible design decision will be fixed in the not too distant future.
For clarification, regarding credit cards, I have 3 company files that use them with 3 different realm ID's. All of them were created using the create new company feature. Company A has Chase and AMEX, B has AMEX only, and C has AMEX and Citibank. In the case of the AMEX across all 3 companies, they do have the same AMEX login which makes sense since they are all tied to me and they have to have a person associated, not just the companies.
I have talked to so many people at tech support and none of them had a clue. Hopefully the server team can fix this problem so it doesn't happen again and hopefully they can give instructions to tech support of how to handle this for all customers that encounter this again.

QBO is terrible, constantly changing with their "improvements" that actually behave like malfunctions. The constant changes are like when you get your office completely organized and efficient and know exactly where everything is and then you leave for the evening and someone comes in and moves all your stuff around. Even if they were actual improvements (which they're not) the fact that everything is constantly changing means you're constantly having to figure out what changed and how to get something done that was totally easy yesterday but no longer functions today. My latest surprise is that you can no longer select text and edit it on line items. When you try to do so it grabs the whole line item and tries to move it. Want to delete a whole paragraph description... you now have to place your curser at the end and hold down delete until each letter is deleted one by one. Completely insane - no other software would edit text like this ever!
@IntuitBrooks Well that was short lived... now this morning AMEX and Chase are back to giving an error. I've attached a screen shot but this is how this all started last summer. So apparently that bug hasn't been addressed in the better part of a year. Any thoughts from you or anyone else on the team?

Based on the screenshot, American Express is updating its online connections and requires users to deactivate and reactivate their online connections.
To fix the bank connection of your AMEX bank to QBDT Mac, ensure your financial institution's list is up to date. Follow these steps below to manually update your bank:

@DebSheenD @IntuitBrooks I followed your instructions @DebSheenD despite the fact that I had serious doubts it would do anything as it worked fine last night... and now we are back to where we were before yesterday and it says that there is nothing to connect because all the accounts are already connected. So now @IntuitBrooks I guess needs to go back to the server team and make them do whatever they had to do to clear things again. Hopefully since they know what to do now it won't take 3 weeks again.
Someone, likely on the server team, needs to figure out 1) why the connections work and then they don't with no change by the user and 2) why when you disconnect the account it leaves records lingering so you cannot reconnect again.
Having things work 1 day out of ever 9 months is not acceptable.

You obviously didn't read... This bug has been going on for around 9 months and @IntuitBrooks so far has been the only person that pays attention and is actually helpful. Unfortunately @IntuitBrooks works on the app team so can't control where the bugs actually are but thankfully tries to be as helpful as possible by communicating internally.
@FishingForAnswers I'm guessing don't read or don't care enough to bother reading... So far @IntuitBrooks seems to be the only competent and caring individual at Intuit.
@dunwoodysteve 20 clams says that the next one replies with something along the lines of 'We appreciate that you've already tried our recommendation to contact customer support, but we still suggest that you do so again.'
Ok @dunwoodysteve we've cleared the connections on the server again. Please go ahead and reconnect. If it stops working let me know here but don't do anything else - I'm hoping we can have a look at it and maybe find the underlying issue.
@IntuitBrooks Something is still wrong, everything is giving the same, already connected message. Tried AMEX on 2 of the 3 files and got the same.

Ok, don't know. Asking the guy with the server hookup.
Did one of the company's work? We only cleared the one. We'll clear all three. When you reply tell me which realms are working. Just give the last four digits of the realm in the message to keep it private. Also for a while leave the Amex accounts off of it and lets see if we can narrow it down to just those. It sounds like you are using Delegate cards - maybe the issue is with that.
Be sure to turn off online banking in all your accounts in all three company files before reconnecting. Just leave Amex delegate out.
@IntuitBrooks So to confirm, I am using Delegate for AMEX as that was the most recent instruction I had.
One company does work with AMEX and is connected now. That company only has the 1 credit card.
One company with Chase and AMEX, both do not work. ID ending 3046
I tried QBDT on AMEX and that never finished.
The other company, Citi works but AMEX still does not and that ID ends in 0306.
I'll go back and turn off the AMEX for the one that works though.
Let me know what next...
Thanks - I'm trying to isolate the problem. My guess is it is the Amex account and if you leave that off you may be able to use all the other ones. This would isolate the issue to just the Amex one. I guess right now just use all of the ones you have working and use webconnect (downloadable .qbo files from the Web site) for the Amex ones that aren't working. Lets just see if that continues to work without problem for a few days.

Hi dunwoodysteve - I know its been a while but I believe the team have figured out the issue with online banking you encountered. My understanding is someone will reach out to you with a test build to try for confirmation.
@IntuitBrooks Thanks for the update, I was about to inquire again wondering what was happening. I'll be on the look out for a contact on this.
@IntuitBrooks They finally got to me Friday and I was able to clear the outstanding items from the web download on Saturday and start testing. I have found that the bank connections now finally work after nearly 9 months and appears to be stable, thanks for that! I've also not had it crash saving an invoice with reminders open, so that is good as well.
I have had it crash on my when I switch from one company file to another around the time it initiates the backup. It's done this more than once though not a lot and I've submitted the normal email reports when it does. Let's hope that doesn't continue as you then need to go back to complete the backup.
I also found that every time you switch files it logs you out of intuit account which is a pain since you can't download without logging back in and the password window does not cooperate with the OS like most everything else allowing you to use the apple keychain to enter the password. If you quit and get back in without switching companies, you remain logged in. This is a new behavior and not a good one.

Glad the banking is working - that's what we are seeing now, too. It was only an issue with multiple different credential sets (log ins for OII) which is what you were seeing. Unfortunately the fix requires that we log you out of the old one when switching which is what you are seeing that requires re-logging in. It doesn't remember the password and both of those behaviors are out of my control I'm afraid.
The crash on backing up isn't something I've seen. Maybe try some File->Back up on its own, without quitting, and see if that works without crashing? I'm betting it will and the crash is related to quitting somehow. Maybe that darned reminders window?
@IntuitBrooks It's interesting that it has to log out and back in even though it's the exact same account. If that's the case, then using the apple keychain (or similar), whether remembering with use permission, or having to re-authenticate each time with computer password or touch ID, would at least make this a faster and more secure process. When people have the ability to use something like touch ID they are more likely to have different and more secure passwords for every app/site. Without that people tend to have highly simplified and reused passwords. At this point QB is one of the very few that doesn't work with this type of approach.

The realm is different (company ID) and that was the issue - we were caching it to avoid relogging in but the security framework was developed by another team and they didn't understand the multiple company Mac use case (they were more iOS focused I think). Yeah, I'm with you on keychain etc. We'll talk to the other team and try to get them to handle it better and prioritize the work.
