Credit Card Authorization Forms

Are we required to use the downloadable Quickbooks customer authorization form when storing credit cards on Quickbook Desktop; or are we allowed to edit it/use our own, one which is more suitable to our company's needs so long as the customer signs & dates it and we do not remove the portion at the top giving us permission to store it?

Additionally, you have the option to download the authorization form from Intuit or use your company's own version of the authorization document.

Ok but do we have to use the Quickbooks provided authorization forms or can we use our own version of a similar authorization form?

Yes, you can create your own version of a similar authorization form, provided it includes all the necessary information that allows you to retain and use your customers' credit card information. Doing so will help protect both your interests and those of your clients.
