Can you tell us what is the originating bank? Is the third-party bank added in the Apps Center in QBO?
Since the check was created or processed by a third-party bank, I would recommend reaching out to them to check why the transaction wasn't honored.

I didn't choose anything. I set up bill payment, selected "pay online" and the rest was supposed to be taken care of. My QBO bill payment history shows completed transaction with check # etc. QB customer support passed me off to Melio who allegedly handles payments. Unable to contact them (recorded message and then disconnected).
