We haven't received any similar reports about the logo field is showing as black on the forms, and I appreciate you taking the time to report this issue. I found if you change the name of your logo to a .GIF the problem goes away. QBO documentation indicates JPG are okay, however, although they appear fine electronically, mine printed as a black box regardless of printer or settings.
